About The Role

We are seeking an experienced Palliative Care Nurse Practitioner or Palliative Care Nurse Specialist to join our specialist nursing team and help shape, strengthen and advance palliative care practice across our residential aged care homes.

This is far more than a traditional clinical role. As a trusted subject matter expert, you will work alongside leaders, clinicians, care staff, residents and families to support complex clinical decision-making, build capability within teams, and champion best-practice palliative care across the organisation.

You will be a visible and approachable presence across our homes, developing meaningful professional relationships and providing guidance, education and support that helps others grow in confidence and capability. Through your expertise and influence, you will contribute to better experiences and outcomes for residents while helping our teams deliver exceptional care when it matters most.

As Our Palliative Care Nurse Practitioner/Specialist, You Will

- Provide specialist clinical assessment, consultation and support for residents with palliative and end-of-life care needs.
- Support the implementation of Eldercare's palliative approach and evidence-based clinical practice.
- Build confidence, knowledge and capability within clinical and care teams through coaching, mentoring and education.
- Facilitate palliative care needs rounds and multidisciplinary discussions.
- Partner with GPs, specialist providers and multidisciplinary teams to support quality resident outcomes.
- Promote advance care planning and resident-centred decision making.
- Provide support and guidance to teams caring for residents with complex symptoms and changing care needs.
- Contribute to quality improvement initiatives, projects and service development opportunities across the organisation.

About You You are a highly experienced palliative care clinician who is recognised not only for your clinical expertise, but for the way you work with others. You are approachable,

supportive and genuinely enjoy helping people develop their skills and confidence. You understand that strong relationships are fundamental to achieving positive outcomes and are able to influence others through collaboration, trust and expertise.

You Will Bring

- Current AHPRA registration as a Nurse Practitioner or Registered Nurse.
- Extensive recent experience in adult and/or older persons palliative care, including the management of complex clinical presentations.
- Nurse Practitioner applicants must hold current NP endorsement and be able to practise autonomously within their scope of practice.
- Demonstrated ability to provide specialist clinical leadership, consultation and support.
- A passion for mentoring, coaching and developing others.
- Exceptional interpersonal and relationship-building skills.
- The ability to navigate complex clinical situations with professionalism, empathy and sound judgement.
- A collaborative approach and the ability to build credibility with stakeholders at all levels.
- Confidence working independently while recognising the value of partnership, communication and shared accountability.
- A commitment to continuous improvement and evidence-based practice.
- Willingness to travel across Eldercare sites as required.

Why Join Eldercare? At Eldercare, you'll be joining an organisation that is deeply committed to excellence in palliative care and recognised nationally for its innovative approach. while contributing to the ongoing growth and development of palliative care services across our organisation.
