Mentis Assist is a not-for-profit community mental health organisation supporting people to improve their wellbeing and live meaningful lives. We are seeking an experienced Case Manager to deliver support across HACC-PYP, Linkages and Transitional Housing programs in Victoria, Australia.
You will conduct holistic assessments, coordinate services, advocate for client outcomes and facilitate transitions to longer-term supports such as the NDIS, My Aged Care and social housing pathways.
