PowerConnect Care is seeking an experienced and dedicated NDIS Support Coordinator to join our team.
Position: Casual
Location: Sydney, NSW
Working Arrangements: Flexible hours and work from home available.
Responsibilities include:
- Helping participants understand and implement their NDIS plans.
- Coordinating supports and liaising with service providers.
- Monitoring NDIS funding and budgets.
- Supporting participants to work towards their individual goals.
- Attending participant home visits, meetings and appointments when required.
