Home Caring are looking for an NDIS/Aged Care Case Manager for the Mornington Peninsula area. We offer a hybrid role of working from home and in the community. We are looking for someone who has experience as a Case Manager in both the Disability and Aged Care sector.

Key Responsibilities:

- Conducting client assessments and developing person-centred care/support plans
- Coordinating services, monitoring outcomes, and reviewing care plans regularly
- Advocating for clients and ensuring their cultural, linguistic, and health needs are met
- Supporting clients to access appropriate services and resources
- Liaising with families, support workers, and health professionals
- Ensuring compliance with the NDIS & Aged Care quality standards
- Enrolling and management of Home Care Packages for Aged Care clients and NDIS funding for Disability clients

What We’re Looking For:

- Case Management experience.
- Experience in aged care, disability, or community health settings.
- Strong communication, problem-solving, and organisational skills.
- Understanding of NDIS and/or Aged Care systems.
- Empathy, cultural awareness, and a passion for helping others.
- Current AHPRA registration (for RNs.)
- Current First Aid & CPR, National Police Check, and NDIS Worker Screening




- Driver's Licence and access to reliable vehicle.
- Full working rights in Australia and COVID-19 vaccination

The Case Manager will support a growing number of clients in the Mornington Peninsula and surrounding area. This role is a hybrid role and is a mix of WFH traveling to clients in the community across and in the office coordinating associated administration and engagement events. Mornington, Mornington Peninsula & Bass Coast VIC (Hybrid)
