Warrigal Home Services is seeking a dedicated Care Partner to coordinate and manage client care in the Southern Highlands region. You will balance multiple caseloads while ensuring services align with individual care plans and approved frameworks.

You will collaborate with clients, families, service providers and internal teams to deliver protected, productive care, focusing on outcomes and wellbeing. A Diploma or Certificate IV in related fields is required.
