Hunter Recent England Local Health District is seeking a Registered Nurse for the Transitional Aged Care Program at Wallsend Health Campus, 38 hours per week. The role delivers person-centred nursing care, supports rehabilitation, and coordinates with a multidisciplinary team to help older people recover after hospital stays.

You will assess, monitor, and respond to clinical changes, advocate for patients and their carers, and ensure culturally secure care aligned with NSW Health CORE values.
