At Home Together in Canberra is seeking a compassionate Aged Care Coordinator / Care Partner to oversee care journeys for an allocated portfolio of clients. ENs or RNs with community aged care experience are preferred to lead assessments, care planning and regular reviews.

You will collaborate with clients, families, healthcare specialists and support workers to deliver protected, person-centred in-home services under the Support at Home program, while maintaining records and ensuring compliance.
