You will deliver secure, patient-centred care in home and community settings as part of a multidisciplinary team, coordinating care and responding to complex needs.

The role requires current AHPRA RN registration, robust clinical skills in community nursing, and a commitment to evidence-based practice with a focus on safety and quality improvement.
