Western Health in Melbourne's west is seeking a part time Disability Navigator to join the Disability Liaison Service.
You will be the first point of contact for people with disability and their families, providing navigation, advocacy and support to access health and community services.
You will collaborate with clinicians, allied health staff and community partners to coordinate complex care pathways, promote inclusive practice and remove barriers to access.
Lived experience is valued.
