Hospital in Your Home (HiYH) provides hospital-level care in the community, supporting hospital avoidance, early discharge, and community-based rehabilitation.
We are seeking an experienced Community Physiotherapist to deliver physiotherapy services under our restorative frailty models of care.You will work in patients' homes and community settings, supporting functional recovery with mobility, balance, strength and falls prevention, while collaborating with a multidisciplinary team.
