This position encompasses a variety of roles including information provision; guided referrals; facilitation of peer support groups; community outreach; developing carer capacity and resilience; delivering practical and innovative practical support.Key ResponsibilitiesUndertake holistic assessments of clients' need and develop goal-directed care plansRespond to emergency, practical and emotional needs of carersCoordinate service delivery and refer to other service types as requiredContribute skills, knowledge and expertise to promote community awareness and understanding of carer issues through outreach and promotional activitiesProviding carer coaching and support to carers to reach their goalsWhat we are looking forRelevant qualification in Social Work, Welfare, Aged Care, Mental Health, Disability or Community Case Management, with an understanding of the needs of carers and the people they support.Excellent communication and interpersonal skills, with the ability to build effective relationships and work collaboratively as part of a team or independently.Robust organisational and digital skills,
