Macedon Ranges Health, supported by Benetas, is seeking an Assessment Officer to deliver in-home HACC-PYP assessments for younger people under 65 in the Gisborne area. You'll evaluate needs, develop care plans in collaboration with clients and families, and coordinate referrals to Health and Home Care services to support independence.
Based in Gisborne, you'll join a multidisciplinary team, contribute to consumer directed care, maintain records, and meet program guidelines.
