Holstep Health is seeking an Assessment and Planning Officer to deliver holistic carer assessments and goal-directed care planning within a hybrid, Preston-based role. You will support access to respite and other services, working across extended hours to match the Carer Gateway program.
You will coordinate with My Aged Care and the NDIS, participate in a statewide multi-disciplinary team, and maintain robust client records. A social work or psychology background is essential.
