You will work with families in homes and community settings, assessing health needs, shaping care plans, and delivering nursing interventions within a trauma-informed, multidisciplinary framework.
You will support families to access health, developmental and disability services, including the NDIS, while promoting child safety and wellbeing through team-oriented case
