DCC Industry Group is seeking a T2A Coordinator to assist young people aged 15 to 18 as they move toward adulthood and independence in Queensland.
You will work across residential, foster and kinship care, and disability services to ensure transition planning starts early and leads to practical outcomes.
You will guide readiness assessments, keep the young person's views central, and coordinate with care teams and external services to turn goals into tangible support.
