You will lead a high-performing team of Transition Care Coordinators to ensure seamless discharge planning and ongoing community support.

The role requires a tertiary qualification in Nursing or Allied Health, strong leadership and stakeholder engagement skills, and a passion for delivering high-quality aged and community
