The First Nations Health Liaison supports Aboriginal and Torres Strait Islander patients, families and communities accessing Grampians Health services across acute, sub-acute, community and mental health settings. Travel between Grampians Health campuses and communities across the region may be required.
Working with multidisciplinary teams, Aboriginal Community Controlled Organisations and other services, the role promotes culturally secure care and positive health outcomes in line with the ICAP
