We are looking for a compassionate, qualified, and experienced Registered Nurse (RN) to join our team and deliver high-quality community nursing services to eligible clients.

Position Details

- Position: Registered Nurse - Community Nursing
- Organization: Activate Care
- Location: Greater Geelong, North, East, South & West Geelong
- Employment Type: Casual / Part-Time

Key Responsibilities

- Provide community nursing services to eligible clients in their homes and community settings.
- Conduct nursing assessments and monitor clients' health conditions.
- Develop, implement, and review individualized nursing care plans.
- Deliver wound care, medication support, catheter care, and other nursing interventions within your scope of practice and authorized service arrangements.
- Maintain accurate clinical documentation and nursing records.
- Liaise with general practitioners, allied health professionals, families, and other relevant stakeholders.
- Ensure compliance with professional nursing standards, clinical guidelines, and organizational policies.




- Promote clients' dignity, independence, safety, and overall well-being.

Essential Requirements

- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.
- Relevant nursing qualifications and clinical experience.
- Strong clinical assessment, communication, and documentation skills.
- Ability to work independently and manage community visits effectively.
- Current First Aid and CPR certification, as required.

Desirable Experience

- Experience in community nursing, home-based care, or wound management.
- Experience supporting older people, people with complex health needs.
