Grampians Health Ballarat is seeking a Permanent Part time Aboriginal Health Liaison to support ATSI patients, families and communities across campuses including Ballarat, Horsham or Stawell. Travel between sites may be required to strengthen relationships and promote culturally safe care.
You will collaborate with multidisciplinary teams and ACCHOs to promote understanding, coordinate referrals and support informed decisions about care in line with the ICAP Strategy.
