Department of Justice and Community Safety VIC in Melbourne Western Suburbs is seeking an Assessment and Transition Coordinator – Mental Health to support prisoners with complex mental health needs as they prepare for life after release.

You will assess risks and needs, develop release plans, coordinate care with prison health services, hospitals and community providers, and lead practical sessions while maintaining accurate records to improve services across Victoria.
