Experienced NDIS Support Coordinator (Level 2) – Hybrid

WellVibe Pty Ltd | Registered NDIS Provider

Location: Wantirna, Melbourne VIC

Employment: Permanent Full-Time | Hybrid Working

Benefits: Company Laptop & Phone | Employee Assistance Program | Ongoing Training & Career development

Your experience. Your judgement. A meaningful difference in someone's life.

At WellVibe, we know that great Support Coordination involves much more than organising services or completing reports. It requires a genuine understanding of the person behind the NDIS plan, the ability to navigate complex funding and service arrangements, and the professional judgement to find practical solutions when challenges arise.

We're looking for an experienced NDIS Support Coordinator (Level 2) who understands the NDIS system thoroughly, communicates confidently with participants and stakeholders, and can independently manage a caseload while maintaining high standards of service delivery and compliance.

This is an opportunity to join an established registered NDIS provider, enjoy the flexibility of hybrid working and contribute to meaningful outcomes for participants across Melbourne.

A minimum of 3 years' direct NDIS Support Coordination experience is mandatory. Candidates with 4 or more years' experience are strongly encouraged to apply.

About WellVibe

WellVibe is a Melbourne-based registered NDIS provider delivering a range of disability and community support services, including Support Coordination, Supported Independent Living, community participation and individualised supports.

We work with participants with diverse disabilities, psychosocial support needs and complex personal circumstances.

Our approach is built on meaningful relationships, practical support, clear communication and respect for individual choice and independence.

We believe experienced Support Coordinators should have the confidence to make informed decisions, the flexibility to manage their responsibilities effectively and the opportunity to contribute their knowledge to improving participant outcomes.

The Opportunity

As a Level 2 Support Coordinator, you'll take responsibility for an allocated caseload of NDIS participants, assisting them to understand and implement their plans, connect with suitable services, monitor their funding and work towards their individual goals.

You will be expected to manage your workload independently, maintain strong relationships with participants and stakeholders, and identify emerging issues before they develop into significant problems.

You will also work collaboratively with the WellVibe team while maintaining appropriate professional boundaries, impartiality and participant choice.

Key Responsibilities

NDIS Plan Implementation & Funding Management

- Interpret NDIS plans, funding categories, budgets, stated supports and applicable funding arrangements.

- Develop individualised plan implementation strategies aligned with participant goals, preferences and circumstances.

- Assist participants to understand their plans, funding responsibilities and available service options.

- Coordinate appropriate disability, allied health, community, housing and mainstream services.

- Monitor plan utilisation, service delivery and funding sustainability throughout the plan period.

- Identify funding concerns, service gaps and changing support needs, and work proactively towards suitable solutions.

- Support participants with changes in circumstances, plan reassessments and the collection of relevant supporting evidence.

Participant Engagement & Wellbeing

- Build and maintain professional, respectful relationships with participants and their nominated support networks.

- Conduct regular participant check-ins and maintain meaningful communication throughout their NDIS plan.

- Undertake in-person wellbeing and support check-ins approximately every three weeks for each allocated participant, taking individual circumstances, preferences and support needs into account.

- Identify potential safeguarding concerns, emerging risks and barriers affecting participant wellbeing or goal achievement.

- Support participants through service disruptions, complex coordination challenges and changes in their personal circumstances.

- Promote independence, capacity building, informed decision-making and genuine choice and control.

NDIS Reporting, Documentation & Compliance

- Prepare accurate and timely plan implementation reports, progress reports and supporting documentation for plan reassessments.

- Maintain comprehensive case notes, participant records, service agreements and relevant coordination documentation.

- Apply current NDIS legislation, relevant Rules, NDIA operational guidelines and applicable funding arrangements.

- Demonstrate sound understanding of the NDIS Code of Conduct, relevant Practice Standards, participant rights and safeguarding obligations.

- Maintain confidentiality, professional boundaries and appropriate information-sharing practices.

- Identify, disclose and appropriately manage actual, potential and perceived conflicts of interest.

- Ensure participants are supported to make independent decisions about their service providers, including where WellVibe offers other disability supports.

Stakeholder Coordination & Problem-Solving

- Communicate professionally with the NDIA, plan managers, allied health professionals, guardians, families and service providers.

- Coordinate multidisciplinary services and facilitate effective communication between relevant stakeholders.

- Assist participants in navigating complex service arrangements, funding questions and changes in support requirements.

- Exercise sound professional judgement when resolving coordination issues and escalating concerns.

- Maintain accurate records and meet agreed reporting and service delivery timeframes.

Hybrid Working With Meaningful Participant Contact

We recognise that effective Support Coordination requires a balance of focused administrative work, professional communication and regular participant engagement.

This position offers a hybrid working arrangement, allowing suitable coordination, reporting and administrative duties to be completed remotely while maintaining regular contact with participants and the WellVibe team.

As part of our participant engagement approach, you'll conduct in-person wellbeing and support check-ins approximately every three weeks, with additional visits or contact arranged according to participant needs, preferences and emerging circumstances.

A company laptop and mobile phone will be provided to support your day-to-day work.

We are looking for someone who can manage this flexibility responsibly, maintain excellent communication and consistently meet participant and reporting requirements.

What We're Looking For

This is an experienced position requiring strong technical NDIS knowledge, excellent professional judgement and the ability to work independently.

Essential Requirements

- Minimum 3 years' direct experience working in NDIS Support Coordination, preferably delivering Level 2 Coordination of Supports.

- Demonstrated experience independently managing an allocated NDIS Support Coordination caseload.

- Comprehensive working knowledge of the NDIS Act, relevant NDIS Rules, NDIS Code of Conduct, applicable Practice Standards and current NDIA operational guidelines.

- Strong understanding of NDIS plan implementation, funding categories, plan budgets, pricing arrangements and funding utilisation.

- Practical experience with PACE, relevant NDIA portals, implementation reports, progress reports and plan reassessment processes.

- Proven ability to interpret NDIS plans and coordinate appropriate services for participants with diverse and complex needs.

- Experience supporting participants with psychosocial disability, complex support arrangements or multidisciplinary care requirements.

- Excellent written communication, professional documentation, stakeholder engagement and problem-solving skills.

- Strong organisational skills, attention to detail and ability to manage competing priorities and deadlines.

- Sound understanding of participant safeguarding, confidentiality, professional boundaries and conflict-of-interest management.

- Ability to work independently in a hybrid environment while maintaining consistent participant engagement.

- Current NDIS Worker Screening Clearance.

- Valid Victorian driver's licence and capacity to travel for participant visits across Melbourne.

- Full Australian working rights.

Highly Regarded

- Four or more years of direct Level 2 NDIS Support Coordination experience.

- Qualifications in Social Work, Disability, Community Services, Mental Health, Psychology, Allied Health or a related discipline.

- Experience navigating complex NDIS funding matters, service transitions, housing-related support needs and plan reassessments.

- Strong understanding of Victorian disability, health, housing and community service networks.

- Experience supporting participants with complex psychosocial needs and multidisciplinary service arrangements.

What WellVibe Offers

- Permanent full-time employment with a competitive annual salary plus superannuation, subject to experience and applicable employment conditions.

- Hybrid working arrangements, providing flexibility alongside regular participant engagement.

- Company-provided laptop and mobile phone.

- Access to an Employee Assistance Program (EAP).

- Ongoing NDIS training, professional development and opportunities to strengthen your knowledge.

- Support from an established management and disability services team.

- The opportunity to independently manage your caseload and contribute ideas to improve service delivery.

- A professional environment that values communication, accountability, participant choice and meaningful outcomes.

Who Will Thrive in This Role?

You’ll be someone who understands the NDIS well enough to confidently navigate funding questions, service gaps, reporting requirements and changing participant circumstances.

You take ownership of your responsibilities, communicate clearly and follow through on commitments.

You’re comfortable working independently, but you also recognise the importance of collaboration, professional boundaries and seeking guidance when appropriate.

Most importantly, you understand that quality Support Coordination isn't measured simply by the number of referrals made or reports completed. It’s about ensuring participants understand their options, receive appropriate supports and make meaningful progress towards their goals.
