The Salvation Army’s Bridge House Withdrawal Unit in Highgate, WA is seeking a part time Care Coordinator (Enrolled Nurse) to support participants through withdrawal care. You will coordinate admissions and discharges, liaise with GPs and external providers, and maintain clinical records to ensure seamless patient journeys.

You will work with a multidisciplinary team, contributing to service quality improvements and ensuring person-centred care within a 0.8 FTE role reporting to the Nurse Team
