What you’ll do

- Build and maintain professional, participant-focused relationships with a small group of participants.
- Understand each participant's preferences, routines, goals, and aspirations.
- Coordinate and facilitate supports that enhance independence and quality of life.
- Act as a proactive problem solver, identifying opportunities to improve participant outcomes.
- Provide a responsive and professional service that focuses on participant experience.

Essential Requirements

- Certificate III or IV in Individual Support, Disability, Community Services,



or equivalent (Support Workers).
- Diploma of Nursing and current AHPRA registration (Enrolled Nurses).
- Current NDIS Worker Screening Check.
- Current Working With Children Check.
- Current First Aid and CPR Certificate.
- National Police Check.
- Robust communication and interpersonal skills.
- Ability to lift 10kg unassisted.
- Experience in Concierge model a bonus but not essential.
