The Metropolitan Remand Centre in Melbourne seeks an Assessment and Transition Coordinator Mental Health to support prisoners with complex mental health needs as they prepare for release. You will coordinate care, collaborate with health teams and community services, and lead sessions while maintaining thorough records.

There is a fixed-term, full time position until 30 June 2027 with salary ranging from $81,496 to $98,955 plus super.
