NSW Justice Health & Forensic Mental Health Network is seeking an Aboriginal Health Practitioner/Worker for Primary Care in Broken Hill. You will provide holistic, culturally appropriate health services to Aboriginal and/or Torres Strait Islander clients within the correctional setting.

The role emphasises prevention of chronic disease and transitions of clients back into the community, working with multidisciplinary teams to ensure seamless care and robust community connections.
