The Royal Melbourne Hospital is seeking a Home Support Assessor to perform non-clinical assessments for older people in their homes, by phone or telehealth, to determine eligibility for the Commonwealth Home Support Programme (CHSP) and link them with appropriate aged care services.

You will collect information using the IAT, create individualised support plans, and help identify services through My Aged Care, working under clinical supervision to build your skills.
