The Royal Melbourne Hospital's Aged Care Assessment team is seeking a Home Support Assessor to help older people access support and services to live well at home and in the community.

You will complete non-clinical assessments in homes, by phone or via telehealth, using the Integrated Assessment Tool and develop individualised support plans to connect clients with My Aged Care services under supervision.
