About Somerville Community Services
Somerville is a for-purpose, community-based organisation that has supported Territorians for over 60 years. We specialise in high-quality disability services that empower people to live the life they choose. As a registered NDIS provider, we deliver individualised support across the Northern Territory, with a focus on respectful, person-centred relationships that promote choice, dignity, and independence.
About the role
We're looking for an experienced clinical professional to coordinate care for participants with complex health needs across our Supported Independent Living (SIL) services.
Reporting to the Clinical Care Lead, you'll manage a caseload of participants, undertake clinical assessments, develop care plans and coordinate complex and high-intensity supports. You'll also work closely with support workers, operational teams and health professionals to provide clinical guidance, training and practical support.
Based in Darwin, this is a hands-on, Monday-to-Friday role involving regular visits to our SIL houses and travel to Palmerston, Katherine and Alice Springs. It offers the opportunity to remain clinically involved while moving away from shift work.
What you’ll do
- Conduct clinical assessments and develop, implement and review participant care plans
- Coordinate complex and high-intensity supports, including hospital discharges and care transitions
- Monitor participant health needs and respond to changes in care requirements
- Visit SIL houses to review care practices and provide clinical guidance to staff
- Deliver practical clinical training, competency assessments and coaching to support workers
- Work with health professionals, families and operational teams to coordinate participant care
- Identify and manage clinical risks, respond to incidents and maintain accurate clinical records
- Contribute to clinical reviews and improvements in care practices
What you’ll bring
- Registration as a Registered Nurse or Allied Health Professional with AHPRA (or relevant professional body)
- Experience delivering or coordinating clinical care for people with complex health needs in disability, hospital, aged care or community settings
- Knowledge of complex and high-intensity supports, including enteral feeding, catheter care, diabetes, seizure management and mealtime support
- Experience providing clinical guidance, training or coaching to frontline staff
- Robust clinical assessment, problem-solving and decision-making skills
- Excellent communication, organisational and documentation skills
- Current (or ability to obtain) Ochre Card, Criminal History Check, NDIS Worker Screening Clearance, First Aid and CPR, NT Driver's Licence and relevant vaccinations
Experience in Supported Independent Living or other NDIS-funded services would be highly regarded.
