Monash Health in the southeast suburbs of Melbourne is seeking a Senior Clinical Case Manager for Adult Mental Health. The role is full time, based at Dandenong Hospital and the Community Service Building, with possible secondment discussions.

You will apply advanced mental health nursing practice, conduct community and emergency department assessments, coordinate care with families and carers, and deliver intensive outreach to diverse clients in the community and hospital settings.
