SA Health's Eyre and Far North Local Health Network is seeking a part time Home Care Coordinator to support older people in the Elliston and Wudinna areas.
You will coordinate Commonwealth-funded home support programs, develop individualized care plans and work with multidisciplinary teams to ensure safety and wellbeing.
The role is a temporary, long-term contract to 27 November ****, offering pro rata ASO3 salary and benefits, including salary packaging, relocation assistance, wellbeing support
