Life Without Barriers seeks a Case Manager – Next Step Plus to support young people transitioning from out-of-home care in Toowoomba and surrounding Darling Downs.
You will manage a caseload, deliver outreach-based, strengths-based case management, and help navigate housing, education, employment, health, wellbeing and independent living.
The role is a six-month maximum-term parental leave replacement with travel across the service area and collaboration with a supportive team.
