Each in Ferntree Gully, Victoria, is seeking a Registered Nurse to join our Health@Home team to support people living with chronic and complex health conditions at home. You will help clients navigate the health system and stay connected to their community.
You will coordinate care, build solid relationships with clients, carers, GPs and providers, conduct assessments and develop tailored care plans, and provide health education to improve wellbeing and health outcomes.
