The role involves initial intake, care plan development, and delivering client-centred in-home care, including transport, shopping, and social support.
You will work across CRM & CMS systems, with travel to client homes and community activities.
Essential qualifications include Certificate III Aged Care and a valid Australian driver's licence; a roadworthy vehicle
