BaptistCare NSW & ACT is seeking a Care Facilitator for At Home Services to support older Australians across Sydney's Inner West and surrounding communities. This full time role combines care coordination, service planning, and community-based work to help clients live independently at home.

You will visit clients, develop and evaluate care plans, work with families and multidisciplinary teams, and document progress in care management systems.
