APM is seeking a Clinical Aged Care Assessor to identify needs of older Australians and connect them with services for independent living. You will travel throughout your service area daily to meet client needs and support where it’s needed most.
You will work in a multidisciplinary setting, performing thorough assessments, referrals, and promoting function while engaging families and carers. The role supports compliance with Department of Health contractual requirements.
