Are you an experienced disability services skilled who enjoys leading teams, coordinating supports, and maintaining high-quality outcomes for Participants?

Hope & Care Community Services is an established, grassroots organisation dedicated to supporting people with disabilities to live independently and participate fully in their communities.

As we continue to expand, we are seeking a Care Coordinator to oversee service delivery across Supported Independent Living (SIL) homes across Brisbane’s Northside, ensuring services are safe, well-coordinated, and aligned with NDIS Practice Standards.

Key responsibilities include:

- Service Delivery Coordination: Coordinate service delivery across SIL homes to ensure consistent, high-quality supports.
- Participant Intake & Transitions: Support Participant intakes, transitions, admissions, and exits.
- Site Oversight & Compliance: Conduct site visits to ensure compliance, safety, and service quality standards are maintained.
- Staff Leadership & Support: Lead and support staff through supervision, training, and performance management.




- Documentation & Incident Management: Maintain compliance incidents reporting, documentation, and Participant records.
- Stakeholder Engagement: Build and maintain strong relationships with Participants, families, and key stakeholders.

You’ll thrive in this role if you:

- Have experience coordinating services in NDIS, healthcare, or community services
- Are organised, proactive, and confident managing multiple priorities
- Demonstrate strong leadership and communication skills
- Understand NDIS Practice Standards, compliance, and service delivery requirements
- Are passionate about delivering high-quality, person-centred supports
