St Vincent's Health Australia is seeking an experienced Registered Nurse to join the National Care Coordination Centre as a Care Coordinator (RN) in Kangaroo Point, Brisbane. You will manage patient journeys from referral to discharge within cutting-edge virtual and home-based care pathways.

You will coordinate services, assess clinical needs, support protected patient navigation and collaborate with medical officers, nurses and external providers in a growing national program.
