SA Health's Limestone Coast Local Health Network in Mt Gambier is seeking a Community Care Coordinator to support community-based aged care, disability and home support services. You will coordinate, implement and evaluate services to help consumers stay independent at home.

Working in a multidisciplinary team, you will supervise staff, ensure quality and compliance, and build effective relationships with health specialists, government agencies and community providers.
