Bayside Health is seeking a Grade 1 Social Worker to support patients in General Medicine at Sandringham Hospital. The role focuses on identifying needs, coordinating care with a multidisciplinary team, and facilitating transitions from hospital to the community.

You will work within the Social Work Department, develop community linkages, and collaborate with internal and external partners to improve patient outcomes.
