Calvary Health Care Kogarah in NSW is seeking an experienced Social Worker to join the Community Palliative Care Team. Plan, coordinate and deliver high-quality social work care to patients/clients in line with NSW Ministry of Health and SESLHD policies.

You will work within a multidisciplinary team, require AASW eligibility, WWCC, NCRC, and demonstrate solid communication, qualified judgment and commitment to digital technologies and quality improvement.
