Bayside Health in Melbourne is seeking an experienced HARP RN to join the Complex Care team at the Alfred Hospital site. The role involves phone risk assessments, triaging referrals, and holistic health assessments to promote patient independence.

You will work with the Alfred case managers, GPs, carers and other services to support self-management of chronic disease and reduce hospital readmissions.
