Australian Unity Limited is hiring a Care Partner to support customers to live safely and independently at home or in preferred settings. The role is permanent full time with versatile schedule, a hybrid mix of client visits, branch days (Ballina, Lismore or Casino) and two days WFH.

You will build trusted relationships, develop holistic care plans, coordinate multidisciplinary services and support customers through transitions while maintaining accurate records and ensuring person-centred care.
