Gladstone Community Linking Agency (GCLA) in Gladstone, QLD seeks an NDIS Care Coordinator to help people live independently by coordinating care plans and connecting them with the services they need.

You will develop and manage care plans, coordinate services, monitor progress, and liaise with families, plan managers and healthcare specialists. The role includes NDIS/Aged Care compliance, budgeting and advocacy to promote wellbeing.
