We are a small, values-driven team focused on quality care, strong clinical support, and meaningful community-based outcomes.About the RoleWe are seeking Enrolled Nurses and experienced Disability Support Workers available for afternoon and evening shifts and active night) for current and upcoming opportunities across the Northern Gold Coast and Logan regions.This is not high-volume agency work, you will:Work with consistent participantsSupport complex and high-intensity needsBe supported by experienced cliniciansDeliver care in participants' homes and community settingsKey ResponsibilitiesDeliver safe, person-centred care to participants with complex needsAssist with personal care, daily living and community accessFollow care plans and positive behaviour support strategiesMonitor and report changes in client conditionComplete documentation (progress notes, incident reports)Build strong,
