Recovery Alliance Care - Greensborough VICWe are looking for an experienced Client Care Coordinator to join our team in a Casual role five Days a week.You will have the flexibility to work from home and from our Office in Greensborough.You will be:Responsible for planning, developing and coordinating servicesResponsible for interpreting participants' NDIS plan,Assisting participants in the planning and implementation of servicesHelping participants gain skills to allow them to become independent in managing their own supportsMonitoring and reviewing plans and goals in consultation with participants on an ongoing basis.Arrange any necessary assessments that participants may require.ABOUT YOU:Have an enthusiastic and compassionate natureExcellent communication and interpersonal skillsHave experience as a Client Care Coordinator or a similar role working within the NDIS settingMinimum of less than 1 year experience is required.Mental Health experience essential.Ability to liaise and build rapport with participants, families and support workers.Able to show knowledge of NDIS and its frameworkExperience with both mental health and disability participantsExceptional time management and organisational skills with the ability to priorities competing demands.Hold NDIS Screening CheckNDIS Screening Check can be obtained after hiringCurrently hold or willing to obtain a National Police Check and Working with Children CheckA valid driving license and access to a vehicle is requiredIf you feel like you need a challenge don't let this chance slide by.Very competitive pay rates.If you have any question feel free to contact Juliana on ******** ***.
