Grand Pacific Health is seeking a Care Navigator for the Medicare Mental Health Centre Moruya.
This fixed-term, part time role (19 hours per week) is based in Moruya and runs until 30 June ****, working to connect consumers with community services and supports to enable integrated, person-centred care.
The role includes concierge duties and requires engaging with consumers and families to provide care navigation and referrals.
