St Vincent's Care at Home is seeking an experienced Registered Nurse to join the National Care Coordination Centre in Fitzroy, Melbourne.
The role focuses on coordinating care pathways, triaging referrals and supporting patients through virtual and home-based services to ensure protected, person-centered outcomes.
You will collaborate with Medical Officers, care teams and referrers, managing patient flow, assessments and documentation within digitally-enabled care platforms.
