Monash Health is seeking a Grade 2 Social Worker to join the Kingston Centre team on a full-time basis in a subacute hospital setting.
You will provide psychosocial assessment, counselling, crisis intervention, advocacy and discharge planning, while coordinating access to NDIS and My Aged Care services.
