Matilda Nepean Hospital is seeking an experienced full time social worker to join our team.Working closely with the entire Multidisciplinary team including a full time discharge planner.

Criteria:
AASW Registration
Minimum of 1-2 years of post graduate experience
Experience working in Mental Health, Rehabilitation and or Palliative Care.
Experience managing clients with complex needs
Experience managing NCAT, NDIS, MAC and other services.
Demonstrated understanding of evidence informed practice and critical thinking
Sound verbal and written communication skills
Ability to work independently and as part of a cohesive team
