The Department of Health, Tasmania, seeks a Clinical Nurse Specialist – Rapid Access Service (CNS-RAS) for the Older Persons Mental Health Service Northwest Community Team in Burnie. You will provide advanced assessment, intervention, and expert consultation for older adults with complex mental health needs and dementia-related symptoms.
Work within a multidisciplinary team to triage, coordinate care, and support staying at home, collaborating with GPs, residential aged care homes, and hospitals
